Use to make changes (pay, fund and cost center, FTE, appointment extension, separation, etc.) to a current employee's job. Please note: appointment extension applies to benefit-eligible faculty or staff only.

Directions to obtain this form:

  1. Send an email to hr-hris@wmich.edu requesting a faculty, staff or temporary transaction form. Please place "transaction form request" in the subject line and include the following information in the body of the email: Employee name, employee identification number, position number (if available), and GL combo code (fund and cost center).
  2. The transaction form will be sent to the requester via email. Please print and complete the form and then route for signatures.
